Leveraging Local Resources for Local Care
Paralign Health operates a health information and payments platform that enables fire and EMS agencies to deliver Mobile Integrated Health–Community Paramedicine (MIH-CP) services under contracts with Medicaid, Medicare Advantage, and ACA healthplans. The system manages patient identification, outcomes tracking and claims submission or payment—allowing payors to fund in-home care through structured reimbursement models.
The Problem:
Healthcare infrastructure in rural America is failing. Faced with physician shortages, hospital and practice closures, and a lack of support, residents are relying on interventions that can’t turn them away: 911 and the ER. We are setting out to support this last line of defense in rural America and give them the resources to provide the right care, at the right time and place.
The Result:
Fewer avoidable 911 calls and ER visits, with aligned infrastructure for scalable, community-based care delivery.

Rural Healthcare Infrastructure is Crumbling
-
Due to a nationwide physician shortage, growing accessibility challenges, and a reactive approach to health care many Americans do not regularly access primary care. The impact is significant as regular primary care access saves $721 in healthcare cost per person, resulting in over $80B in avoidable healthcare costs annually
-
More than 60M Americans live in rural areas where healthcare access challenges are exacerbated due to less providers and regular hospital closures - resulting in 23% higher mortality rates and 1.5x suicide rates
-
EDs across the nation are overcrowded and our 911 systems have become largely a medical response system (Over 70% of 911 calls are medical, and largely non-emergent). Many Americans in rural areas use 911 and the ED in absence of a primary care provider. Driving costs up and placing excessive burden on our 911 systems and EDs, resulting in stress on EMS/Fire agencies and our hospitals and providers.

Paralign Health connects local Fire/EMS agencies with regional payors to stand up and sustain Mobile Integrated Healthcare & Community Paramedicine programs.
The Solution
EMS services are quite limited in their scope—they respond to emergencies and stabilize patients. By integrating with a healthcare entity (like a payor), EMS can stand up MIH-CP programs and provide a wider scope of healthcare services. These programs can decrease strain on the emergency response system and local ERs—and reduce the unsustainable costs they face—by providing more appropriate and preventative care.

MIH-CP Programs Across the Country are Showing Results
-
MIH-CP programs have proven results, in Missouri MIH has shown a massive reduction in cost after 30 months of patient enrollment
-
MIH-CP Pilots in rural Colorado demonstrate significant return on investment as MIH-CP program shows decreased ER utilization
-
MIH-CP programs in Oklahoma have shown the ability to drastically decrease overuse of the 911 system by high utilizers via timely intervention and behavior change methods
